Insurance Waiver

Please find below our Insurance Waiver, to be filled out if you’ve never played at an FAO site before. Insurance waivers are valid for 6 months, if you have gone over this period of time you must resubmit a completed insurance waiver in order to play.

PLEASE NOTE: Age 12-16 participants must have an adult stay on site
